Community Support Service
Action Deafness aims to support Deaf, Hard of Hearing, Deafened and Deafblind people to gain independence thus achieving their full potential. Action Deafness will deliver a specialist package of community support to those people living in their own homes and local communities, who require this service.
Agreements may include liaison with Deaf/Hard-of-Hearing Service Users, Leicestershire Adult & Social Care's Services to Deaf and Hard of Hearing People, specialist mental health services or learning disability teams in Leicester, Leicestershire and Rutland.
Action Deafness Community Support Service will enable Deaf, Hard of Hearing, Deafened and Deafblind people to maintain their independence in a range of different ways. Action Deafness will work in partnership with the referrer as well as other agencies involved with the individual client to ensure an effective multiagency approach.
Community Support Workers will engage with the client through the individual support plan process. This system will enable a package of support to be created, agreed between Action Deafness and the client as well as other agencies involved with the individual. This focused approach will ensure that strong links are forged between all agencies involved with the client in order to provide the most appropriate support package.
An agreement will be created with input from all parties, with specific emphasis on placing the client at the centre of this process. The support worker will enable the client to forge links with their own local community and encourage increased independence through a range of varied activities.
